Starts off in the 15th century, with Connor McLeod training with another immortal swordsman, the Japanese sorcerer Nakano. When an evil immortal named Kane kills the old wizard, the resulting battle leaves him buried in an underground cave. She lies on the bed and her breasts are visible. She gives him a condom saying, "No cuv, No love." The man, unsure as to what it is, tries to eat it. He approaches her, rips off her thong, and it looks like they are having sex. Several sexual references ensue. A graphic sex scene of a man and woman having sex, woman's breasts with erect nipples are shown. Man puts his mouth on her left breast. Then he pulls off her thong and her pubic hair is shown. He puts his hand in her crotch. Woman is seen totally nude, as she is on top of man, having sex and her butt is shown. Several more references to male genitalia.

A man is shot in an alley. He lays down in a pool of blood. (but survives) A man is killed via guillotine during the French Revolution. A security guard is killed by the antagonist off screen. A man gets decapitated and his head speaks for a few moments before dying. There is no blood and this scene feels very silly despite trying to be serious. At the beginning, the villain Kane and his henchmen slaughter an entire village of people. Conner slices Kane in half during their final battle, but Kane laughs and puts himself back together. Conner cuts off Kane's head. It bobbles around on his neck before falling off and bouncing down surrounding steps. His decapitated body falls over and undulates a bit before going stiff. Probably the most graphic kill in the Highlander Film Series.
